Patient Navigation and Community Health Worker Training

 

The Patient Navigation and Community Health Worker Training program delivers high-quality, interactive trainings to navigators and community health workers (CHWs). PNCT's full curriculum prepares trainees for the state of Colorado CHW credentialing exam. Training topics include health promotion, motivational interviewing, professional boundaries, care coordination, cultural responsiveness and resource connection.

Navigators and CHWs help clients “navigate” the healthcare system, obtain timely care and get connected to community resources. They often have a close connection and understanding of the communities they serve. Navigators help foster health for all by serving populations in need.
